To protect people from preventable infectious disease that cause needless death, we, (Justin Bintliff) can administer the following vaccines to eligible patients 18 years and older, according to indications and contraindictions in current guide lines from the Advisory Committee on Immunization Practices of the Center for Disease Control and Prevention and other competent authorities.

Influenza Virus Vaccine (Fluvirin® or Afluria®) 0.5 mL
OrigHuman Papillomavirus (Gardasil®) 0.5 mL
Hepatitus A (Vaqta®) 1 mL
Hepatitus B (Engerix-B® 20mcg/mL) 1 mL
Pneumonia (Pneumovax® 23) 0.5 mL
Cyanocobalmin Vitamin B-12 1 mL
Shingles Vaccine (Zostavax®) 1 mL

Vaccines and Immunizations

In the course of treating an adverse effect following immunization/vaccination, we, (Justin Bintliff) are authorized to administer the following medications pending the arrival of emergency medical services:

Epinephrine (~0.01 mg/kg/dose (maximum 0.5mg/dose) SQ or IM

Diphenhydramine
(~0.01 mg/kg/dose (maximum 50-100mg/dose) PO or IM

The pharmacist will maintain current certification in cardiopulmonary resuscitation capability.
